New Client Information

FairView Counseling specializes in Play Therapy. All of our therapists have gone on to become
Registered Play Therapists or Board Certified Art Therapists after earning their Master’s or Doctoral Degrees.

Expectations

We ask that you please arrive 15 minutes prior to your initial session with your completed forms.

You must bring two signed consent forms from your intake packet with you at the time of your intake appointment, or you will not be seen for your scheduled appointment.

Frequently Asked Questions

What if I cannot get a signed “consent for treatment” from my child’s other parent?

Exceptions include an order of sole legal custody, court-ordered treatment, or death certificate; all would require documentation on the day of intake. If you are unsure if your documents qualify, you should fax them to the office prior to your appointment. If you are not in contact with your child’s other parent, consent for treatment can be handled through a lawyer and/or the stand-alone Consent for Treatment (above) can be mailed/faxed separately to the office prior to your appointment.

How do I know what therapist is right for me?

To begin counseling services, complete our online form. If you have a therapist preference, please indicate that on the form. You will be matched with the therapist who best meets your needs based on your concerns, insurance participation, and availability.

What happens in the first session?

During your intake appointment, your therapist will meet with you to discuss your current situation and concerns, obtain a background history, and explore what you are hoping to accomplish by coming for therapy. She will explain how therapy will work and help you agree on treatment goals. For clients 13 and under, the first session will just be parent/s/guardian/s (no child). For clients 14 and over, the client is expected to be at the first session.

FairView Counseling requires our clients to follow the immunization guidelines recommended by the American Academy of Pediatrics.

Payment Information

FairView Counseling accepts most insurance plans, some Pennsylvania CHIP programs, and some EAP’s.

For our clients who do not have insurance, or their insurance does not cover our services, we offer payment using a Sliding Fee Scale which is determined by household income. More information is available about this plan by contacting our office.
