As a new client, there is some preliminary paperwork that you’ll need to complete and bring with you to your first appointment. We ask that you please arrive at least 15 minutes prior to your initial session with your completed forms to complete additional paperwork.

FVC requires a signed ‘consent for treatment’ from BOTH PARENTS REGARDLESS of marital status. You must bring two signed consent forms with you at the time of your intake appointment, or you will not be seen for your scheduled appointment.
What to expect
How do I know what therapist is right for me?
When you call FairView Counseling, one of our intake specialists will gather all the necessary information including your concerns, insurance information, and your availability. The Clinical Director will then match you with a therapist that is best suited to meet your needs.
What happens in the first session?
During your first appointment, your therapist will meet with you to discuss your current situation and concerns, obtain background history, and explore what you are hoping to accomplish by coming for therapy. She will explain how therapy will work for you/your family and help you agree on treatment goals.
For clients 13 and under, the first session will just be parent/s/guardian/s (no child). For clients 14 and over, the client is expected to be at the first session.
